I was thinking whether or not there was a free tool online that would allow me to make a fillable PDF form and I found one at pdfamigo.com. It's free and pretty easy to use. Basically you click "Start Now" and use the menus to add your form fields (text field, radio buttons, list box, etc.). You can add text, images, and multiple pages and then download the file and do with it what you please, send to a colleague, post it for students.
